Leave the engine alone. You will beat your head, and chase your tail investing time and money trying to make a little more power, when the answer is inside of the differential housings.
Change the gears, and obtain a mechanical advantage over your current gears, and really feel a seat of the pants improvement.
Consider your overall ratio based upon the tire size, and then consider how much easier the truck will accelerate with a better ratio.
The difference will be night and day, and you will not have to worry about reliability or the emission problems.
You will have a point and shoot truck that will be very very impressive with a bone stovk engine. This will be far less work, and will fit into the current budget.
Horsepower costs money, and serious power is very expensive. Trying to increase power from an engine that has 100K on the clock is not real practical.
The gear change is a win, win situation.

Okay...what gears are you running on your truck?? Hey 75, I have the same truck and was looking for a decent seatg of the pants change...not to hijack the thread, but I am running 3.73 gears on my truck with stock wheels but with 285/75 BFG AT's. They are the same height as the stockers (265/75/16) but a little wider. So do I go up in numerical value...say 4.10's? Is this what this guy should do depending on what gears he is running? I like the gear change idea....

you have a truck and you are worried about gas milage, but even if you have this truck as a project vechile you might only drive it on weekends so what does it matter, i would go with 4.10's and get some bigger tires like some 285's

and you r the guy with the Little Red Chevy right?
