TBi had an issue with vacuum leaks under the TBi pretty common. If not that then troubleshoot your system, Starting with fuel pressure.

have you watched the injectors on startup?? i dont believe that them being dirty is going to be the main cause to your problem, although a good cleaning of the butterflys and throttle body itself is probably in order. let the truck sit for a while and then try to start it. watch the injectors to see when they start spraying fuel meaning, do they start spraying immediately or is there a hesitation there. might also turn the key to on, let the fuel pump pressure completely up and then starting it to see if there is any quicker reaction time to the engine starting on its own.

let me explain this so you know where i am going with this. those injectors will not work until a certain PSI is reached. if you are jumping in and starting it without letting the pump charge the line fully, this can cause it to turn a few times without starting. this would indicate a fuel line leaking down. by waiting for the pump to fully charge the line, you will narrow down your problem.
also take silvers advice. i have heard good things out of seafoam and this can only help your injectors out.
