sporadic miss

'91 GMC with '92 350 misses sometimes when it is started. May miss for 1 to 3 miles so bad it will hardly go up a hill. I put new plugs, cap, rotor, coolant temperature sensor on. Wires are good. Sometimes it starts and runs great. Never misses when a mechanic or tech is around. I'm wondering about the ignition module or pick-up coil. Any ideas?

Could be your EGR or IAC valve, (Idle Air Control)

check for vacuum leaks
sparay soapy water on plug wires, watch for cracking noise.
spray carb cleaner on vacuum lines and gaskets, if it bogs from some area - you got your culprit.
definitely clean EGR and PCV.

I had a few of those trucks in my fleet. Question is what does the intake look like and is it Throttle Bottle Injected. Other is, any codes coming up.
